Sunnyside Community Services (SCS) is a community-based nonprofit centered on the belief that every person deserves meaningful support to achieve their aspirations. SCS serves a diverse community with programs that include home care and home health aide training, a vibrant senior center, and a full range of services for at-risk older adults, including social adult day care for individuals with Alzheimer’s, and a city-wide program of supportive services for those who care for them.
The Social Adult Day Program at Sunnyside Community Services offers specialized support and therapeutic activities for older adults with dementia. The Social Adult Day program provides a way for loved ones with memory loss to continue living at home while providing respite for their caregivers. A low staff-to-participant ratio ensures participants receive the individualized care and attention they need. A kitchen, dining room, and activity area creates a home-like environment. Certified aides assist participants with personal care and medication reminders. Participants engage daily in activities designed to meet their needs, interests, and abilities, including yoga, music, art, games, and pet therapy.
If you or your loved one is having difficulty with daily activities, Sunnyside Community Services provides compassionate attention, available 24/7. In addition to providing home health aides, older adults and their families can rely on support—from filling out an application for Medicaid long-term care to ensuring safety at home to counseling families when a loved one is losing their memory. Help is available twenty-four hours a day, seven days a week. Home health aides speak more than twenty languages, including Spanish, French-Creole, and Tagalog. Home health aides receive ongoing training to improve their skills, including specialized coursework in Alzheimer’s and dementia care. Clients experience better-controlled pain, higher medication adherence, and significantly fewer falls with injury. Sunnyside Community Services can assist you to obtain Medicaid or create a long-term care plan.
